Shareholders are requested to intimate any change in their postal address, e-mail address, signature or bank account details to the Registrar and Share Transfer Agent, KFin Technologies Limited (“RTA”) of the Company by furnishing Form No. ISR-1 and other forms pursuant to SEBI Master Circular No. SEBI/HO/MIRSD/SECFATF/P/CIR/2023/169 dated October 12, 2023.
The same are available herein below and the duly filled-in Forms may be sent to RTA at einward.ris@kfintech.com through the registered e-mail id of the shareholder or can also be submitted in hard copy to the RTA.
Form ISR 1 – Request for Registering PAN, KYC details or changes/updation thereof
Form ISR 2 – Confirmation of Signature of securities holder by the Banker
Form ISR 3 – Declaration Form for Opting-out of Nomination by holders of physical securities in Listed Companies
Form ISR 4 – Request for issue of Duplicate Certificate and other Service Requests
Form SH-13 – Nomination Form
Form SH-14 – Cancellation or Variation of Nomination
Annexure C – ISR 5 – Request for Transmission of Securities by Nominee or Legal Heir
Annexure D – Individual Affidavits to be given by ALL the Legal Heirs OR Legal Heirs named in Succession Certificate
Annexure E – Bond of Indemnity to be furnished jointly by all Legal Heir(s) including the Claimant(s)
Annexure F – No-Objection Certificate from the Legal Heir(s)
Form A – Affidavit Cum Indemnity For issuance of duplicate securities
